If so, Health Connect America is looking for a dedicated and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in empowering individuals to achieve their mental health and well-being goals through community-based support and intervention.

What Will I Be Doing:
As a Community Support Specialist, you will be at the forefront of providing essential care and assistance to at-risk youth and their families. Your primary responsibilities will include: Individualized Support, Community Integration, Emotional Support, Skill Building, Crisis Intervention, Documentation, Team Collaboration.

Qualifications:

  • A Bachelor's degree in Psychology, Social Work, Counseling, or a related field.
  • Two years experience working with kids and families.
  • Compassion, empathy, and a genuine desire to support individuals on their mental health journey.
  • Strong communication and active listening skills.
  • The ability to work independently and as part of a team.
  • Knowledge of community resources and social services.
